Planning for Your Assessment 

Your assessment begins before you arrive at Pain to Performance. Our preparation process is purpose-led make sure we have all the information and materials beforehand, to conduct a thorough assessment and build a plan that addresses your specific needs.

Every prospective client starts with a screening call.

Planning for Your Assessment

The assessment process begins after you have completed the initial screening call and we have determined that Pain to Performance Center appears to be an appropriate fit.

Your assessment is completed across three pre-scheduled appointments:

  1. A three-hour in-person assessment

  2. A 30-minute report and planning consultation

  3. A 90-minute in-person follow-up exactly one week after the assessment

Once you have completed the screening call, enrolled in the service, and scheduled your assessment, you will receive detailed preparation instructions by email.

Before Your Appointment

 

Please plan to:

  1. Complete your intake forms carefully and return them by the stated deadline.

  2. Avoid waiting until the day before the assessment to complete your intake forms.

  3. Bring your lumbar MRI study in a DICOM CD-ROM format 

  4. Bring any relevant surgical reports or medical documentation requested during scheduling.

  5. Wear or bring comfortable clothing that allows us to observe and assess the spine, hips, and lower extremities.

  6. Bring comfortable walking shoes.

  7. Allow the full three hours for the assessment. 

  8. Avoid scheduling another commitment immediately before or after the assessment.

Pain and Anti-Inflammatory Medication

Medication used to reduce low-back or leg symptoms may temporarily alter the responses we are attempting to assess.

Do not discontinue or change prescribed medication against the advice of your medical provider. However, please understand that taking pain-relieving or anti-inflammatory medication shortly before the assessment may limit the clarity of the findings.
 

Continue taking medications prescribed for other health conditions as directed.

After You Enroll

​Your confirmation email will provide more detailed instructions, including:

  1. Intake-form deadlines;

  2. Required records and imaging;

  3. What to wear and bring;

  4. Arrival information;

  5. Preparation for the Plan & Education Teleconference; and

  6. What to expect during the 90-minute follow-up.
