Referral Overview
Pain to Performance is a self-pay, non-medical service that provides the McGill Method mechanical-based assessment for low back pain and associated leg symptoms, in addition to education, and coaching service for selected individuals who fit one of the five established referral pathways.
The current referral process is primarily patient-initiated. A physician or clinician may recommend Pain to Performance and direct the individual to the Request Appointment form. Pain to Performance then reviews the individual’s history, current medical status, previous care, symptoms, functional limitations, and goals before determining whether an assessment is appropriate.
A medical referral or prescription is not automatically treated as confirmation that the individual is appropriate for enrollment. Every prospective student must complete the Pain to Performance screening process. Pain to Performance does not require the referring office to manage the individual’s enrollment. The individual remains responsible for completing the Request Appointment and screening process.

Confirm the Pathway Fit
Before providing Pain to Performance’s information, consider whether the
individual appears to fall within one of the established pathways:

Pre-surgical gray-zone assessment;

Persistent symptoms following conservative care;

Imaging and clinical presentation discordance;
-
Post-surgical or post-PT rebuild;
-
Peturn to work, fitness, or sport.
The individual should already be established within an appropriate medical pathway and should not be presenting with findings that require urgent medical or surgical management.

The Process
1. Provide the Individual With P2P Information
The physician or clinician provides the individual with Pain to Performance’s website, referral information sheet, or Request Appointment link.
2. The Individual Submits a Request
The individual completes the Request Appointment form and provides information regarding symptoms, medical history, previous care, current status, and functional goals.
3. Pain to Performance Screens the Inquiry
Pain to Performance reviews the submitted information and conducts an initial phone screening to determine probable pathway fit, medical readiness, and whether an in-person assessment should be scheduled.
4. Enrollment and Communication Are Established
When the individual is accepted, enrollment requirements are completed. The individual identifies the physicians or clinicians with whom Pain to Performance may communicate and authorizes the release of relevant reports and progress information.
NOTE: Once communication authorization has been established, Pain to Performance may correspond with the identified medical or rehabilitation professionals regarding assessment findings, progress, limitations, or concerns relevant to the individual’s pathway.
ATTENTION: When the presentation does not fit Pain to Performance’s scope or requires further medical evaluation, the individual will be directed back to the appropriate medical or surgical provider.

Self-Pay Information
Pain to Performance is a self-pay service and does not bill commercial insurance, Medicare, or Medicaid.
A clinician’s recommendation does not create insurance coverage or guarantee enrollment. Program suitability, costs, scheduling requirements, and payment policies are reviewed directly with the individual before an assessment is confirmed.
