Before You Submit
Our services may be appropriate for those are seeking help with:

Chronic, recurrent, or unresolved low-back or hip-region pain;

Leg or foot symptoms with or without accompanying low back pain;

Rebuilding after spine surgery;
-
Returning to exercise, work, fitness, or sport after a history of back pain; or
-
A presentation that has not responded durably to generalized care
You should also be prepared to participate actively, complete the required intake process, and follow a structured assessment and follow-up timeline.

After your form is submitted:

We will review the information provided.

You may be contacted for clarification when needed.

When the service appears potentially appropriate, you will be offered a screening call.

The screening call is used to determine readiness and the most appropriate pathway.

An assessment is scheduled only after the screening process is completed.
